Tag-mode panel shows wall of duplicate text

Spam handling settings page repeats the entire explainer 27 times — once per domain. Plus every domain shows "Tagging enabled" when scanner actually has tag off.

Redesigned: single explainer at top, compact one-line row per domain with status badge, advanced thresholds in collapsible details. Also fixed fallback values to read from system_settings instead of hardcoded 0.85/0.50.
